  • Police report, if dispute involves unauthorised or fraudulent transactions
  • Email or SMS alerts/notifications related to the disputed transactions
  • Authorisation form, if account is jointly held
 

2. Credit Card/ Charge Card

  • Credit card statement showing name of the cardholder, card number and the disputed transactions
  • Correspondence (e.g emails, letters or messages) with the financial institution and their responses to your dispute
  • Police report, if dispute involves unauthorised or fraudulent transactions
  • Email or SMS alerts/notifications related to the disputed transactions
 

3. Loan Account (e.g. Housing Loan, Renovation Loan, Business Loan)

  • Loan statement showing name of the accountholder, account number and the disputed transactions
  • Correspondence (e.g emails, letters or messages) with the financial institution and their responses to your dispute
  • Letter of Offer issued by the financial institution (if available)
  • Authorisation form, if the loan is held jointly
 

4. Investments

  • Account/Portfolio statement showing name of the accountholder, account number and the disputed transactions
  • Correspondence (e.g emails, letters or messages) with the financial institution and their responses to your dispute
  • Sales documents such as application forms, financial needs analysis (if available)
  • Product brochure, prospectus or factsheet (if available)
 

5. Electronic Transactions and Payments

  • Account statement showing name of the accountholder, account number and the disputed transactions
  • Correspondence (e.g emails, letters or messages) with the financial institution and their responses to your dispute
  • Police report, if dispute involves unauthorised or fraudulent transactions
  • Transaction receipt, telegraphic transfer application (if available)
 

6. Life Insurance Products (e.g. Annuity, Endowment Plan, Health Insurance, Investment-Linked Policy, Term Life Insurance, Whole Life Insurance)

  • Policy document/contract including the policy schedule, terms and conditions
  • Correspondence (e.g emails, letters or messages) with the financial institution and their responses to your dispute
  • Written approval from company’s Human Resources deparment (applicable only for Group Health insurance policies)
  • Financial needs analysis (if available)
  • Assignment documents, if policy has been assigned

7. General Insurance – Motor Accident Products, including third party claims

  • General Insurance Association (GIA) Accident report/ Traffic police report
  • Police investigation outcome

  • Surveyor report and coloured photographs of the damaged vehicle/ accident scene (if applicable).
  • Rental agreement, invoice or receipt for alternative vehicle (if applicable).
